High School Sports Requirements
Groupings are based off of last name, first letter.
FALL CLEARANCE SCHEDULE Football practices begin August 16 All other sports practices begin August 21
Thur, Aug 10 8 – 3 Football Seniors 8-10 am, Juniors 10-12pm, Lunch 12-1, Sophomores 1-3 pm
Fri, Aug 11 8 – 1 Football (Freshman 8–10 pm, Any Sport 10-12pm, Lunch 12-1
Mon, Aug 14 8 – 2 Boys/Girls Cross Country and Girls Golf A-G =8- 9am, H-M =9-1pm, N-S= 11-12pm, Lunch 12-1, T-Z =1- 2pm
Tue, Aug 15 8 – 2 Girls Swim, Boys Water Polo, and Cheerleaders A-G =8- 9am, H-M =9-1pm, N-S= 11-12pm, Lunch 12-1, T-Z =1- 2pm
Wed, Aug 16 8 – 2 Boys Tennis, Volleyball, and Dance A-G =8- 9am, H-M =9-1pm, N-S= 11-12pm, Lunch 12-1, T-Z =1- 2pm
Thur, Aug 17 8 – 2 Girls Soccer and All Athletes A-G =8- 9am, H-M =9-1pm, N-S= 11-12pm, Lunch 12-1, T-Z =1- 2pm
** There will be NO Clearance on Friday, Aug 18. This is a roster creation day for all Fall Sports.
In order to participate at one of the three district high schools, the following requirements must be met:
1. Complete the digital forms found at https://puyallupschools.rankonesport.com/New/NewInstructionsPage.aspx
- Cautions, Considerations and Responsibilities
- Steroids Flyer
- Athletic Eligibility Form
- Concussion / Sudden Cardiac Arrest Information Sheet
- Parent Permission
2. Submit a current physical examination form to the high school Athletic Director. Most physicals are good for two years. The physical must be valid for the entire sport season. (WIAA #17.11.0) D.C.
3. If a student has a life-threatening health condition or needs access to emergency medications, then an Emergency Action Plan, signed by a physician, must also be submitted. This can be completed on the back of the Clearance Card or attached as a separate sheet.
4. Pay the Athletic Fee ($75 per season) and purchase a high school ASB Card ($40). These fees can be paid in person with the school bookkeeper prior to athletic clearance, or by using the new payment program https://wa-puyallup.intouchreceipting.com/
(a convenience fee is charged). In either payment situation, bring your receipts with you to the Athletic Director.
5. You will need to check in with the bookkeeper to clear any fines you might have on your account before you can clear with the Athletic Director.
Any athlete with fines WILL NOT be cleared.
6. Academic eligibility will be checked by the Athletic Director. Students must pass five of six classes with a 2.0 or higher GPA in the previous semester (spring grades).
7. Get a Yellow Clearance Card from the high school Athletic Director. Bring the signed UNFOLDED purple card to your coach on the first day of practice!!!
Before the first day of practice – Get cleared through the PHS athletic director.
No Gold card? No Practice, No Exceptions!
